Purpose of Meeting
MercyONE, UnityPoint Health, Broadlawns Medical Center and Telligen are organizing a series of community events across Central Iowa for senior citizens and their families/caregivers. These events will provide valuable information on Advanced Care Planning, Fall Prevention, Diabetes Management, Blood Pressure Screening, Care Transitions, Vaccine Education & Screening, and connections to Community Resources and Support. There is no cost to the attendees of the event.
