Purpose of Meeting
MercyOne, Unity Point, Broadlawns & Telligen have planned a community event to provide valuable information on Advanced Care Planning, Diabetes Prevention Programs, Care Transitions, Vaccine Education & Screening, Falls Prevention, Elder financial Fraud & Scams, Blood Pressure Screening, and connections to Community Resources and Social Needs Support Services. There is no cost to the attendees of the event. All of our venues, speakers and exhibitors have volunteered their time and resources.
